How an online request form will be introduced.
The planned first-stage form is limited to ordinary coordination contact details. It is not active yet, because ANYUTRA has not verified all operational facts required to accept even non-sensitive personal information online.
Planned first-stage fields
Contact email; optional country or region; preferred contact method; approximate China visit month or date range; optional preferred language; and selected coordination needs: hospital administrative communication, document translation coordination, interpreter, accommodation, airport pickup, private transport, family support or tours.
Not part of the first form
No medical records, health descriptions, uploads, passport or identity details, payment information, hospital booking request or free-text medical narrative.
What must be verified before the form opens
The accountable data controller and public contact; a complete privacy notice and data-rights route; storage location and retention/deletion procedure; an actively monitored recipient and response process; approved notification provider and failure recovery; access controls and log minimisation; abuse protection; and a real non-sensitive end-to-end test. Until then, the online form remains unavailable and no data is collected through it.
Future document handover
After initial contact, ANYUTRA will explain an appropriate secure document-sharing and consent process before receiving health records. That later process requires separate confirmation of sensitive-data consent, authorised sharing with the customer’s chosen hospital or translation provider, access controls, retention/deletion and cross-border requirements.
